Mounjaro did not arrive in a vacuum. It is the latest product of a research program that has shaped diabetes medicine since 1876 — when Eli Lilly founded the company in Indianapolis, decades before insulin itself existed as a treatment.

  • 1876 Company founded in Indianapolis, Indiana by Colonel Eli Lilly, a Civil War pharmacist who set out to produce drugs of verified purity at industrial scale.
  • 1923 First commercial insulin (Iletin), produced under licence from Banting and Best. Lilly's industrial fermentation made insulin available worldwide within a year of its discovery.
  • 1982 Humulin — the first recombinant-DNA medicine approved for human use anywhere on earth. The synthetic human-insulin platform Lilly built for Humulin is the engineering ancestor of the peptide-manufacturing chain that produces tirzepatide today.
  • 2018 Tirzepatide LY3298176 enters phase-3 trials. The molecule was deliberately engineered as a dual incretin agonist — a chimeric peptide hitting both the GIP and GLP-1 receptors with rebalanced signalling potencies.

In Thailand, both the type-2 indication (Mounjaro) and the engineered manufacturing chain behind it are imported under the same Thai-FDA registration. There is no "generic tirzepatide" — only the Lilly-manufactured product.

Why two receptors out-perform one

Single-receptor GLP-1 agonists like semaglutide bind only the GLP-1 receptor. Tirzepatide co-activates the glucose-dependent insulinotropic polypeptide (GIP) receptor at the same time. That second axis is what separates the SURPASS efficacy curves from everything that came before.

GLP-1 AXIS

Appetite, satiety, gastric emptying

GLP-1 receptor activation slows gastric emptying, amplifies postprandial insulin release, and signals satiety centrally through hypothalamic POMC neurons. This is the axis every GLP-1 drug shares — Wegovy, Ozempic, Saxenda, Trulicity, Victoza.

GIP AXIS · TIRZEPATIDE ONLY

Adipose insulin-sensitivity, lipid handling

GIP-receptor agonism improves insulin sensitivity inside adipose tissue, suppresses lipolysis appropriately in the fed state, and appears to attenuate the nausea ceiling that limits GLP-1 dose escalation — patients tolerate higher exposure.

Head-to-head evidence — SURPASS-2. Frias and colleagues randomised 1,879 adults with type-2 diabetes to tirzepatide 5/10/15 mg or semaglutide 1 mg, all once-weekly, over 40 weeks. Mean body-weight change at week 40 was −7.6, −9.3 and −11.2 kg for the three tirzepatide doses respectively, versus −5.9 kg for semaglutide 1 mg (NEJM 2021).

Receptor-level proof. Coskun and colleagues characterised the LY3298176 binding profile against both GIP and GLP-1 receptors, showing −5× higher affinity for GIP and modestly lower than native GLP-1 for the GLP-1 receptor — the deliberate "imbalanced" signature that gives tirzepatide its distinct metabolic signal (Mol Metab 2018).

Tirzepatide is escalated in 2.5 mg increments every four weeks. Each rung gives the gastrointestinal tract a full month to adapt before the next jump. Most patients reach a steady-state therapeutic dose between 10 mg and 15 mg.

A patient can stay at any step indefinitely once weight goals are met. There is no clinical requirement to climb to 15 mg if the target was hit at 7.5 or 10 mg.

Mounjaro is the right starting molecule for

  • +Patients with type-2 diabetes or pre-diabetes who need both glycemic control and weight reduction — the SURPASS data shows simultaneous improvement on both axes.
  • +Patients targeting more than 20% body-weight reduction. Only tirzepatide and (off-label) compounded peptides have crossed this threshold in published trials.
  • +Patients prepared for a 20-week titration window and willing to inject through dose-escalation discomfort rather than abandoning the protocol at week 4.
WEAKER FIT

Consider a different GLP-1 if you are

  • −Severely GI-sensitive. Tirzepatide's higher exposure ceiling means more nausea in the first 8 weeks for some patients. Saxenda's daily, shorter-half-life profile may be gentler.
  • −Time-constrained on titration. Mounjaro's 20-week climb to maximum is four weeks longer than Wegovy's 17-week pathway. Patients on a fixed return timeline may prefer Wegovy.
  • −Already established on Ozempic or Wegovy with good response. Switching to tirzepatide adds receptor coverage but also restarts the titration clock. Stay on what is working.
