A century of peptide engineering, distilled into one molecule

Wegovy is semaglutide dosed at 2.4 mg weekly. Behind that single specification sits a Danish pharmaceutical lineage stretching back to 1923 — and a deliberate decade of half-life engineering that turned a 2-hour native peptide into a 165-hour weekly drug.

• 1923 Nordisk Insulinlaboratorium founded in Copenhagen by August and Marie Krogh after their licence from Banting and Best. The competing Novo Terapeutisk Laboratorium opened two years later. The two merged in 1989 to form Novo Nordisk.
• Today World's largest insulin manufacturer by volume. Roughly half of all insulin used by people with diabetes globally is produced inside the company's Danish, French and US fermentation sites.
• 2005 Liraglutide (Victoza) — the company's first long-acting GLP-1 analogue. A fatty-acid side-chain anchored the peptide to circulating albumin, extending its half-life to about 13 hours and enabling once-daily dosing.
• 2017 Ozempic FDA approval — semaglutide, an extensively re-engineered second-generation GLP-1 with a half-life of approximately 165 hours. Once-weekly dosing became practical for the first time at this stability.
• June 2021 Wegovy FDA approval — the same semaglutide molecule, up-dosed to 2.4 mg weekly, indicated for chronic weight management in adults with obesity or overweight plus a comorbidity. Based on the STEP-1 phase-3 program.
• Mar 2024 Cardiovascular indication added — Wegovy becomes the first weight-management drug ever to carry a FDA-approved cardiovascular-risk-reduction label, on the strength of the SELECT outcomes trial. See the next section.

The only GLP-1 with a FDA cardiovascular label

If you have established cardiovascular disease and are not diabetic, Wegovy is the only GLP-1 agonist on the market whose label explicitly includes reducing the risk of a heart attack, stroke or cardiovascular death. That distinction comes from the SELECT trial.

The Wegovy 5-step dose ladder

Wegovy reaches its 2.4 mg target dose in 17 weeks — three weeks faster than Mounjaro's 20-week schedule. The titration uses doubling and roughly-doubling jumps every four weeks, with a 1.7 mg intermediate step that flattens the climb to the maximum dose.

Strong fits and weaker fits, stated plainly

Wegovy is not the strongest weight-loss number on a clinical-trial chart — that is Mounjaro. But it carries the longest safety dataset in the GLP-1 weight-management class and the only cardiovascular outcomes label. Decide accordingly.
